On this page you will find everything you need to apply for a Seniors Card as well as forms to order a replacement card and change your address.

How can I apply for a Seniors Card?


Before you apply for your Seniors Card, make sure you check the eligibility criteria. If you meet the criteria, you will need to print out the Seniors Card Application Form (PDF 115 kb) and have it signed by someone authorised to witness a Statutory Declaration. Take along some proof of age, such as a birth certificate, Aged Pension Concession Card or driver’s license. People who can witness Statutory Declarations include local councillors, pharmacists, solicitors, sworn members of the Victorian Police, doctors and Members of Parliament.

Seniors Card application forms are also available from Australia Post and post offices.

I’ve lost my Seniors Card – how can I get a new one?


There are a range of ways you can obtain a replacement Seniors Card – just choose the one you prefer. You can:
  • Complete the Replacement card form. Please allow 2 to 3 weeks for your application to be processed and for your card to be mailed to you.
  • Phone the Seniors Card Program Hotline on 1300 797 210
  • Write to us at Seniors Card Program, GPO Box 4316, Melbourne 3001

I have a Seniors Card – how can I change my address?


Whenever you move, please tell us! We want to ensure that you continue to receive your copies of the directories of discount providers. There are a range of ways you can tell us you’ve changed address – just choose the one you prefer. You can:
  • Complete our Change of Address form
  • Phone the Seniors Card Program Hotline on 1300 797 210
  • Write to us at Seniors Card Program, GPO Box 4316, Melbourne 3001
